Investigation Summary
At 12:30 p.m. on April 12, 2017, an employee was on the roof of a residential home approximately 18 to 23.5 feet above the ground measuring the opening in a chimney for a replacement chimney cap/chase. The employee fell to the ground and landed on his face and lower arms. The employee suffered blunt force trauma to his face and head, fractured his skull, and sustained abrasions and lacerations to the lower forearms and hands. The employee was killed.
