Investigation Summary
At 3:15 p.m. on June 13, 2017, an incident occurred in the parking lot of a fuel station travel center. While there were no witnesses to the incident, it was believed that the employee fell from a flatbed semi-trailer while attempting to spread a tarp over the load of lumber.The employee was transported to a nearby hospital, where he was hospitalized and received treatment. However, as a result of suffering a traumatic brain injury, the employee died on June 21, 2017.
