Investigation Summary
On July 27, 2017, an employee was working between the Large Tapering Machine and the Cutting Area that moved a tube which had been tapered and cut into the finished bin. While the employee moved the tube to the finished bin, a new tube was in the Large Tapering Machine being tapered. The employee was struck in his head when a part of the tube was not secured in a rest and ejected from the machine. The employee was killed as a result of his injuries.
