Investigation Summary
At approximately 11:30 a.m. on November 17, 2017 an employee was up on a roof, at the very top, painting corbels when he started to climb down a step ladder he laid across the top of the roof. As he was climbing down the ladder, the ladder slid down the roof, and the employee fell approximately 9 feet from the edge of the roof to the concrete ground below. The employee sustained blunt force trauma to his head resulting in fatal injuries. The cause of the accident was failure to provide employees with fall protection, improper use of ladders and failure to train employees in the safe use of ladders.
