Investigation Summary
At 1:30 p.m. on February 13, 2018, an employee was working from a scaffold to repair a roof scupper drainage system. The scaffold was constructed with 2 by 4 wood boards and measured four stories high. Also, an A-frame ladder led to an extension ladder that was affixed to the scaffold. After the employee applied water proofing to the scupper and was leaving the scaffold, he fell to the ground and was killed.
