Investigation Summary
At 10:10 a.m. on March 3, 2018, an employee was clearing a jam from a McCloskey International 621RE Trommel. During work, the employee became caught in the conveyor system and was dumped into a trash pile located approximately 150 feet from the equipment. The incident investigation revealed that past practices involved the clearing of jams while the machine is in operation, which it was speculated that the employee followed this same practice. The employee suffered a head trauma from the incident and was killed.
