Investigation Summary
At 3:03 p.m. on March 14, 2018, Employee #1 was opening the valve to recirculate thick beet juice from one tank to another. As the employee stepped back, his leg struck the lever to the sampling ball valve. This opened the valve and allowed the thick juice to spray into the air from the valve. The temperature of the juice was approximately 80 degrees Fahrenheit, with a ph of 9.2-9.5. Employee #1 received burns to his eyes, face and upper body. He was hospitalized for treatment.
