Investigation Summary
At 2:00 p.m. on March 19, 2018, an employee was delivering a load of pipes from a tractor trailer. The employee walked along the side of the trailer and removed two load chains that was holding the pipes in place. The employee was struck by a pipe that fell from the second tier and the employee later died from internal injuries.
