Investigation Summary
At 11:30 a.m. on April 26, 2018, an employee was replacing the chaff collection drum on the after-burn cyclone on Roaster #Z4 and exposed the energized rotary airlock feeder. The employee removed a full chaff collection drum when he reached up into the duct work to clean out any built up chaff and his right index finger came into contact with the unguarded rotating valve of the rotary airlock feeder. The employee amputated his right hand's index finger when caught by the rotary valve rotating inside the valve housing. The employee was hospitalized.
