Investigation Summary
At 9:24 a.m. on May 21, 2018, an employee was operating an IMT drill rig (model AF300) drilling shafts ranging in depth of 12 to 45 feet and 10 foot wide for concrete caissons to an overhead light rail track. The employee finished drilling a 32 foot deep by 10 foot wide shaft and was moving the drill truck to the next drill location. The track on the drill rig came to close to the finished shaft and the ground underneath the drill rig gave way. The drill rig fell sideways and the operator fell out of the rig. The employee was engulfed by dirt. The employee suffocated. The employee had been operating the rig with the door open and not wearing the seatbelt.
