Investigation Summary
At 2:57 p.m. on July 25, 2018, an employee was replacing a transformer inside the eighth floor electrical vault room. A loud explosion occurred, which was heard by nearby coworkers. The employee was then observed staggering towards the door of the vault room and collapsed. A coworker performed CPR on the employee and he was taken to a nearby hospital where he later died.
