Investigation Summary
At approximately 9:00 a.m. on July 30, 2018, Employee #1 was cultivating top soil, using a 1976 Case Tractor, Model number 1210, and Serial number 11151290. A cultivator attachment was being used to assist with the tilling. The wheel track width of the tractor was approximately seven feet, while the wheel track width of the cultivator attachment was approximately 10 feet. The soil pile was approximately 60 feet wide, 200 feet long, and ranged from 0-20 feet high. The slope/height of the soil pile was at approximately 36 degrees and 15 feet in height. The top layer of the soil was tilled to create loose soil that could be sent through a soil shredder to create pulverized top soil. The employee was an experienced operator, working alone. The tractor rolled over the sloped section of the pile, resulting in fatal crushing injuries to the employee. The tractor was not equipped with a roll-over protective structure (ROPS) or a seatbelt.
