Investigation Summary
At 10:30 a.m. on August 10, 2018, an employee was plowing a field with Caterpillar D4 tractor with a disk attachment and drove over the side of an embankment. According to the Coroner's report, the employee sustained crushing injuries and died as a result. There was no evidence that the tractor or attachment rolled over.
