At 11:00 a.m. on September 13, 2018, an employee was working in forming area of a HFW (High Frequency Weld) area cutting pipe. The employee was crushed between pipe cutting machine and a piece of pipe, receiving fatal internal injuries. The pipe was 24 inches in diameter, weighing approximately 94 lbs per ft.
