Investigation Summary
At 2:00 p.m. on December 13, 2018, an employee was working with a coworker and they were attempting to pull five stacked casting skids along a holding rail using pneumatic winches in the blast area of a foundry. The employee had attached an open hook from the north Ingersoil Rand Air Motor, Model EU, Serial Number RCA91059, and had pulled the slack out of the cable which unbalanced the stack. The falling casting skids toppled over on top of the employee, killing him.
