Investigation Summary
At 7:00 a.m. on February 22, 2019, an employee was using a 10 foot extension ladder to access the top of a mobile home in order to install a tarp. As the employee was steadying himself on the ladder by grabbing the fascia of the mobile home, the ladder broke beneath him. The employee fell approximately 11 feet and struck his head on asphalt. As a result, the employee suffered injuries to his head and face, killing him.
