Investigation Summary
On February 26, 2019, an employee was using a 6-foot fiberglass A-frame ladder (Louisville L-3016-06) to access shelved materials in a warehouse. A coworker heard a loud noise in the direction where the employee was working. As the coworker went to investigate the noise, the employee, the ladder, and 2 to 3 boxes were found on the floor. The employee was taken by car to a nearby urgent care facility where he was evaluated, received treatment, and released the same day. The employee returned to the same medical facility the next day, as well as follow-up appointment. Approximately two weeks later, on March 10, 2019, the employee was transported by paramedics from his private residence to the hospital where he passed away.
