Investigation Summary
At 7:15 a.m. on May 15, 2019, Employee #1 and #2 determined that the top loader container handler #45 required a new front tire on the right, due to a pre-inspection that had taken place. The employees began changing the split-rim tire. They jacked the front of the top loader container handler and began to remove the inflated tire. Twenty six of the twenty seven bolts that bolted the tire to the drive axle were removed. As the employees removed the last bolt, the inflated tire's wedge rim separated from the tire's rim with tremendous force. It struck both employees, driving them into the top loader container handler that was adjacent to them, parked in the power shops bay. Employee #1 was killed and Employee #2 was in a coma in a hospital.
