Investigation Summary
At approximately 5:30 a.m. on May 14, 2019, an employee opened a guard/door and reached into China forming machine no. 3 to clear a jam without utilizing lockout/tagout procedures. The forming machine cycled causing the right arm caught in the head of the machine. The employee sustained lacerations to the right forearm requiring stitches and was hospitalized for these injuries.
