Investigation Summary
At 3:03 p.m. on June 6, 2019, an employee and a coworker were operating a forklift that had a pallet of cardboard which the employee was sliding into the bailer. The employee was struck by the bailer when it fell onto him as his coworker pulled on it, killing him.
