Investigation Summary
At 8:00 a.m. on April 22, 2019, an employee was standing on a 6 foot scaffold cutting wood on the side of an entryway overhang roof. A coworker was standing on the overhang roof and was using a powered reciprocating saw to cut out the top layer of the roof. The coworker told the employee to climb up a 4 to 6 foot ladder that was in place on the top of the scaffold, leaning against the roof, to get up on the roof to work. The coworker went back to using a powered reciprocating saw. When he turned back around, the employee was laying on the sidewalk, and the scaffold and ladder had fallen. The employee was taken to the hospital for trauma to the head, where he later died.
