Employee falls into industrial blender and is crushed

On July 26, 2019, an employee was on the top platform and had the lid open to the industrial blender while preparing to run a new color. The employee fell into the blender and his coworker turned on the blender, heard a thump, and turned off the blender. The employee's coworker pulled him from the blender but it was too late. The employee was killed.
