Investigation Summary
At 1:30 p.m. on August 19, 2019, an employee was putting a pipe in a Chiyoda CPC-60 bender machine. The employee was getting ready to make the first bend on a pipe in the bender machine. The employee was holding the pipe to be bent and started the machine. The pressure die closed on the pipe, and the employee's finger was caught between the pipe and the pressure die. The employee yelled for help and a coworker and the general supervisor were able to stop the machine and release the employee's finger. The employee was hospitalized to treat a dislocation, fracture, and laceration to the left index finger.
