Investigation Summary
An employee was working as a licensed vocational nurse (LVN) at a nursing care or skilled nursing facility. The facility specialized in post-acute rehabilitation and the complex care of residents. It had several COVID-19-positive patients and employees during the course of the pandemic. The facility had two COVID-19 cohort units for positive residents during an outbreak at the facility. The employee did not work in the cohort units. On May 29, 2020, she tested positive for COVID-19. At 5:00 p.m. on June 23, 2020, she died from complications as a result of COVID-19. The Division learned of the death on July 2, 2020, after her employer called to report. A Division representative opened an investigation of the fatality on July, 3 2020, and arrive on-site on September 22, 2020, to conduct an on-site inspection of the facility. Through the course of the investigation and interviews with the employer and employees, it could not be determined whether the employee had any direct contact with any of the positive patients or suspected positive patients. It was never determined where the employee contracted COVID-19.
