Investigation Summary
At approximately 12:00 p.m. on September 11, 2020, Employee #1 was at home with pneumonia and COVID-19. Employee #2 was sent home on September 4, 2020, by ER. On August 31, 2020 Employee #1 was sent home because the employee was not feeling well. Employee 1 went to the hospital and tested positive on September 6, 2020. Employee 1 was sent home from the hospital after being told he had pneumonia. Employee #1 died on September 11, 2020, of acute cardiorespiratory arrest and sequentially COVID-19. Employee #2 was sent home from work on September 4, 2020, and tested positive for COVID-19 on September 9, 2020. The employee was hospitalized on September 16, 2020, and was released 6 days later on September 22, 2020.
