Investigation Summary
At approximately 11:45 a.m. on December 8, 2020, an employee was returning to the work area on the production side of the shelving. The employee was a pedestrian at the time. The employee was struck by a powered industrial vehicle in the vicinity of the aisle and shelving area. The employee was struck and pinned between the vehicle and the shelving unit. The employee suffered internal crushing injuries and died from these injuries.
