Investigation Summary
An employee had been working for a landscaping company. The employee had returned back to her Northern California worksite after working at a Southern California worksite when she reported coughing and having COVID-19 related symptoms. The employee last worked on December, 20, 2020. On December 22, 2020, the employee took a test and was positive for COVID-19. Four coworkers had also tested positive for COVID-19. On December 23, 2020, the employee was hospitalized and died as a result her infection on January 29, 2021. The date, time and other details leading up to exposure are not indicated in the narrative. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. The employer did not have an effective COVID-19 Prevention Program in place.
