Investigation Summary
At 5:41 a.m. on August 11, 2021, an employee who worked at a 26-bed psychiatric facility died from COVID-19. The employee's job duties are not indicated in the narrative, but the employer does not care for confirmed or suspected COVID-19 positive patients. All patients arriving at the facility are screened for COVID-19 prior to being admitted. The employee last worked at the facility on July 29, 2021. During that time, a patient started to exhibit COVID-19 related symptoms. The employer immediately took precautions by having employees wear respiratory protection and other personal protective equipment. The patient was moved to an isolated part of the facility awaiting transfer to a hospital within 5 hours. The employer tested all employees at the facility and 3 coworkers who identified as close contacts with the employee tested positive for COVID-19. The investigation was inconclusive to determine if employees were infected by the patient or each other. Employees always maintain social distancing and wear face masks while in the facility. The employer did not have Adequate Aerosol Transmissible Diseases policies and procedures in place.
