Investigation Summary
At 12:00 a.m. on March 19, 2022 an employee, working alone, was infilling a hole in the concrete floor of a lift station. The employee removed the metal grate that was covering the hole in the 6-inch concrete floor and began to drill holes into the side of the concrete to place dowels and formwork for the concrete infill. As the employee was drilling into the concrete at the side of the hole, he lost his balance and fell through the hole to the floor of the pump room. The employee was killed.
