Investigation Summary
At 2:13 p.m. on May 26, 2023, an employee was helping lift a gin lent filter pipe with a CM Series 622 hand chain hoist. The employee climbed two flights of stairs, climbed over the top handrail, and walked out onto a structural I-beam. The employee, then, suddenly fell to the concrete surface that was approximately 8 to 10 feet below. The employee died due to head trauma injuries.
