Investigation Summary
Three employees were "assembling and fitting" a section of a ship. One employee was in an aerial lift, another employee was on a ladder adjacent to the lift, and Employee #1 was at the base of the ladder holding the ladder in place. The employee on the ladder was handing materials from the ladder across to the employee in the aerial lift. The employee on the ladder passed a six-pound sledgehammer to the employee in the lift who put the sledgehammer down on the floor of the aerial lift then reached for another piece. The sledgehammer fell from the lift striking Employee #1 on the hard hat causing him to fall to the ground. Employee #1 received a laceration to the left side of the face and head trauma causing seizure activity to his right arm and leg. Employee #1 was hospitalized.
