Investigation Summary
At 1:37 p.m., on June 15, 2016, three employees were installing a 4 inch sewer line in a 12 ft. trench. The employee a 33 year old male, was inside the trench leveling out the bottom when the walls of the trench collapsed, trapping and killing the employee underneath.
