Investigation Summary
At approximately 11:00 a.m. on July 21, 2003, Employee #1 and coworkers were removing a vertical pump and motor inside a gas plant. For some reason, a hydrocarbon was released inside the plant, causing an explosion and fire. Employee #1 suffered second- and third-degree burns to his upper body. He was hospitalized and treated for his injuries. Employee #1 died ten days later.
