Investigation Summary
At 4:15 p.m. on February 20, 2014, an employee was participating in the filming of a scene on an active railroad trestle. The employee was working with approximately 20 coworkers on the train trestle with cameras and other filming equipment along with a hospital bed that was set-up perpendicularly across the tracks. While filming on the train trestle, a train came on the track. Due to the lack of time and a safe method to exit the trestle, the employee and eight coworkers were not able to exit the trestle prior to the train reaching them and, therefore, huddled on the east side of the trestle on a metal walkway platform. The train struck the bed, causing the employee to be pulled into the train, causing multiple fatal injuries.
