Use this checklist as a tool to help you in your efforts to identify hazards in your facility and keep track of follow-up measures.

  1. Print out the checklist for all of the tasks in this area of your plant to take with you as you evaluate this area of your plant. To use this option, select "Print Blank Checklist" below and a new window/tab will appear with the checklist. Then, using your browser's print option, print the checklist.

  2. Use the online checklist below and make selections and comments before printing. To do this, first check the box next to each hazardous situation that you have identified. Then for each suggested solution, indicate whether the solution is currently in use, something you want to consider, or not applicable. Include comments about the solutions or applicable follow-up procedures. Note that you may wish to identify other solutions that would prevent or control the hazardous situation in your plant. At the end of each task, there is also a field for identifying other hazards related to the task or comments about the task.

    Select "Print Completed Checklist" and a new window/tab will appear with the checklist filled out. Using your browser's print option, print the checklist. The checklist printed with this option will include only the hazardous situations for which you have made entries. YOUR ENTRIES WILL NOT BE SAVED ON YOUR COMPUTER.

Collision

Use signs and policies to prohibit others from entering these areas.

Use horn when appropriate to alert others.

Improve lighting where applicable.

Drive forklift backwards when moving loads.

Defective Forklift

Train operators to inspect and recognize potential problems or hazards.

Implement equipment inspection plan.

Report and correct problems as soon as identified.

Struck by Falling Loads

Institute a program of safe storage and stacking.

Train employees in proper procedures for housekeeping.

Wrap loads in plastic to maintain stability.

Inspect and replace bad floor areas.

Fumes From Battery Charging

Provide all employees with personal protective equipment.

Provide eyewash and showers for emergencies.

Provide proper ventilation.

Hazards From Working With Pallet Jacks

Provide windows in doors low enough for employees to see who or what is coming on the other side, even if pushing pallet jacks.

Train employees in proper and safe methods for using hand/electric jacks, to avoid injuries.

Provide one-way entrance and exits.

Other Hazards

 

Bending and Twisting While Lifting Heavy Loads

Stay within NIOSH guidelines for safe lifting.

Reduce the size of the box or decrease the amount of weight to be lifted.

Provide lift assists, so boxes may be lifted from the conveyor with the assist and placed on the pallet.

Provide palletizers or scissor lifts for employees so they do not need to bend over to place boxes on pallets lying on floors.

Develop a whole new workstation to eliminate elevated and low lifts.

Use a telescoping conveyor to load trucks if loads are not palletized.

Rotate employees to tasks that do not require heavy lifting.

Exposure to Cold Environments

Provide warm clothing that fits properly and does not hinder movement.

Provide adequate periods of rest so employees can recuperate.

Rotate employee to less stressful conditions.

Maintain areas and equipment to minimize slipping.
